861 E. Heritage Parkway, Prairie Grove. Critical violations: Employee was cutting lemons with bare hands. Employee restroom did not have an employee hand wash sign. Raw, uncovered chicken was being stored in the cook prep 1 on a top shelf over ready-to-eat food. Multiple bottles of chemical solutions were not labeled. Noncritical violations: An employee lacks food manager certification.
